Overview

This calculator determines the glucose infusion rate (GIR) from intravenous and/or enteral sources. It supports multiple simultaneous IV lines and enteral feeds, and calculates the combined GIR from all sources.

GIR Formula

GIR is calculated using the following formula:

GIR (mg/kg/min) = [Dextrose % × IV rate (mL/hr) × 1000] / [weight (kg) × 6 × 100]

For enteral feeds, GIR is derived from the carbohydrate content of the formula or human milk at the given feed rate and infant weight.

Clinical Reference Ranges

Glucose infusion rates above 12–14 mg/kg/min are generally considered high and may be associated with hyperglycemia. Minimum GIR requirements vary by gestational age, postnatal age, and clinical condition.

Note: Enteral GIR is an estimate only. No distinction is made between continuous and bolus feeds or between carbohydrate sources. All enteral feeds are assumed to be digested and absorbed.
